• Student needs to be 4 years of age by September 1st, 2025
• Need a copy of the following:
  • Birth Certificate
  • Proof of residence (utility bill with address)
  • 3231 Immunization form
  • Social Security documentation (copy of SS card or fill out this waiver)
  • 3300 Form (Ear, Eye, Dental and Nutrition). This will need to be requested at your child’s Four year old check up.
